For everyone who lives at your address — at least one of:
- Valid driver’s license with current address
- Current utility bill
- Current mail
- Lease agreement including all resident names
Proof of income
Provide one of the following:
- Current pay stub
- Copy of your latest tax return
- Current Disability Income Award Letter
For each child
One of the following:
- Medicaid letter
- CHIP card
- Shot record
- Letter from a doctor
- School enrollment letter
- Crib card for babies
